1. Lodgement of this application does not guarantee your child will be offered a place. If your child is offered a place, we will ask you to complete an enrolment form to formalise your child’s enrolment.

- The information we collect on this form will be used by the ATSICHS Brisbane team for the primary purpose of contacting you to provide information on how your child can attend kindergarten.
- De-identified data provided on this form (not your name, your child’s name, or contact details) will be used to measure kindy enrolments.
- Kindy is free for all eligible aged children.
- By completing this form you agree that your information is able to be given to the ATSICHS Brisbane team so they can contact you to discuss enrolling your child in kindy.
